Welcome to our ASQ Online screening program!

Carousel Academy Families, 

We value your input and your experience with your child.  This is a great tool to help you and us gauge your child's current development and how we can better serve them.  This is just a screening and not a determining factor in your child's "success".  Childhood is a journey, not a race, so we are grateful to be along for the ride.  We plan to use these as a way to better serve your child and family, as well differentiating our instruction to reach EVERY child.  

In the event that we feel your child may benefit from a formal evaluation, we will reach out to you and provide our input.  If you have concerns would like to schedule an evaluation, please reach to your child's teacher.  These screenings would be shared with the district if you provide consent to the evaluation.    We will share results with you at the Parent/Teacher Conference, but if you would like results prior to the November date, please contact Mrs. Sylvis at [email protected].  She can share a pdf and host a phone call to review them with you.  

These screenings are due by: September 16, 2022.

Because your child's first 5 years of life are so important, we want to help you provide the best start for your child. You've been invited to participate in the Ages & Stages Questionnaires, Third Edition (ASQ-3), to help you keep track of your child's development. The questionnaire may be provided every 2-, 4- or 6-month period. You will be asked to answer questions about some things your child can and cannot do. The questionnaire includes questions about your child's communication, gross motor, fine motor, problem solving, and personal social skills.

Please enter your child's birth date and the number of weeks he or she was born premature below to start the screening. Please note that the information you enter into this website is secure and cannot be seen or accessed by anyone other than the program employees who have invited you to participate in this screening.
