Once you start a questionnaire for your child and submit, it will be sent to the Sylvan Lake Family Resource Network program to be reviewed. Once a trained professional has reviewed your child’s results they will contact you with the summary of the questionnaire. At this time you will have the opportunity to ask questions and connect with resources to support your child's development.
