Welcome to our online developmental screening program! As a tool to help monitor your child’s development, you have been given access to this program from your Johnson County Early Learning Collaborative partners (Child Care Aware of Eastern Kansas, Head Start of Shawnee Mission Inc, Infant Toddler Services of Johnson County, Kansas Association for Infant and Early Childhood Mental Health, Kansas Children's Service League Healthy Families, and Parents as Teachers).

Because your child's first 5 years of life are so important, we want to help you provide the best start for your child. You've been invited to participate in the Ages & Stages Questionnaires, Third Edition (ASQ-3), to help you keep track of your child's development. The questionnaire may be provided every 2-, 4- or 6-month period. You will be asked to answer questions about some things your child can and cannot do. The questionnaire includes questions about your child's communication, gross motor, fine motor, problem solving, and personal social skills.

Please enter your child's birth date and the number of weeks he or she was born premature below to start the screening. Please note that the information you enter into this website is secure and cannot be seen or accessed by anyone other than the program employees who have invited you to participate in this screening.

By clicking the “Submit” button below, you are giving consent for Infant Toddler Services of Johnson County to review your screening results.  This review will determine if a more involved evaluation is recommended.  All information will be kept confidential.

After entering in your contact information, you will be asked to fill out an age-specific ASQ-3 questionnaire.  The questionnaire should take 15-20 minutes to complete.  You may need to gather a few items to check whether or not your child can perform a task. You can complete the screening online as you work with your child, or print it off and then login again to enter your results. Please include any other information you feel helpful to understanding your child's development, including any difficulty in understanding your child’s speech or behavioral concerns, in the "Overall" section on the last page.
