Welcome to the Ages and Stages (ASQ) screening!

As your child’s first teacher, you play an active role in the screening process. Your observations, ideas, and concerns about your child are very important and we are asking you to share them with your child’s teacher. The ASQ will screen for developmental, behavioral, language and motor abilities. This screening will take approximately 15 minutes and is designed to be completed by the parent.

Please enter your child's birth date and the number of weeks he or she was born premature below to start the screening. The information you enter into this website is secure and cannot be seen or accessed by anyone other than the program employees who have invited you to participate in this screening.
