IF your child's has an IEP/IFSP (including speech only services)- You do not need to complete the ASQ.
SCREENING INSTRUCTIONS:
- Unless instructed otherwise, select the FIRST option to complete both portions of the ASQ (3 and SE-2)
- Include the name of your child's current Preschool/Child Care program NAME and LOCATION and/or any outside agencies you may receive services through (e.g.Theraplay, Easterseals)
- Review consent (next page)
