Welcome to Developmental Milestones, our ASQ Online screening program!

Is your child learning, thinking, moving and growing at the right pace?

Developmental Milestones can help you better understand and meet the developmental, social-emotional, health and educational needs of your child. Enrolling is especially important if your child had a low birth weight, a difficult or premature birth, or has medical issues.

Because your child's first 5 years of life are so important, we want to help you provide the best start for your child. You've been invited to participate in the Ages & Stages Questionnaires: Social-Emotional, Second Edition (ASQ:SE-2) to help you keep track of your child's social and emotional development. The questionnaire may be provided for use at 2, 6, 12, 18, 24, 30, 36, 48, or 60 months of age. You will be asked to answer questions about some of your child's behaviors. The questionnaire includes questions about your child's ability to calm down, take direction and follow rules, communicate, perform daily activities (e.g., eating, sleeping), act independently, demonstrate feelings, and interact with others.

Please enter your child's birth date and the number of weeks he or she was born premature below to start the screening. Please note that the information you enter into this website is secure and cannot be seen or accessed by anyone other than the program employees who have invited you to participate in this screening.

Once you enter your child's birth date, the system will select a questionnaire based on your child's age (adjustments may be made for premature birth). You can print the questionnaire to review the questions, or immediately enter the responses. Once all questions have been answered, submit your responses online.

By clicking SUBMIT you are enrolling your child in Developmental Milestones monitoring and consenting to on-going screening through the Idaho Department of Health & Welfare Infant Toddler Program. Your responses will be reviewed by Infant Toddler Program staff and you will be invited to complete future developmental screenings for your child. You may request an evaluation of your child's development at any time by contacting the Developmental Milestones Team at 208-334-6530 or [email protected].  This evaluation would be provided at no charge.  You may choose to withdraw from this program at any time by contacting Developmental Milestones at 208-334-6530 or 450 W.
