Welcome ESEP Therapists and Special Instructors!  You are about to access our ASQ:3 Online screening program!

As a reminder, ASQ:3 is a great tool to accurately identify and monitor children at risk for developmental delay from birth through age 5.  While it is designed to identify a child’s strengths and/or areas where a child may need encouragement or support, it isn’t designed todiagnose delays or disabilities or identify specific child goals to target.  As you enter the child’s information, it will automatically for prematurity and account for developmental age.

ASQ-3 concentrates on 5 areas of development:  communication, gross motor, fine motor, problem solving, and personal-social. The ASQ-3 questionnaire may be provided every 2-, 4-, or 6- month period depending on age.

If completing this questionnaire brings up any questions or concerns, please don’t hesitate to contact either of us.
