SCREENING INSTRUCTIONS:  

-Unless instructed otherwise, select the FIRST option to complete both portions of the ASQ (3 and SE-2)

-If your child has an IEP/IFSP, you do not need to submit any screenings

-Include the name of your child's preschool program and/or any outside agencies you may receive services through (e.g.Theraplay, Easterseals).

-If you are completing the ASQ’s for a CEEP peer application, please list “Colonial Early Education Program” as your child’s preschool program in additional to any current programs they are currently enrolled in.

-Review consent (next page)

Welcome to the Colonial School District's Ages & Stages Questionnaire (ASQ). Please answer each question to the best of your ability, and we thank you for your time.

Because your child's first 5 years of life are so important, we want to help you provide the best start for your child. You've been invited to participate in the Ages & Stages Questionnaires, Third Edition (ASQ-3), to help you keep track of your child's development. The questionnaire may be provided every 2-, 4- or 6-month period. You will be asked to answer questions about some things your child can and cannot do. The questionnaire includes questions about your child's communication, gross motor, fine motor, problem solving, and personal social skills.

Please enter your child's birth date and the number of weeks he or she was born premature below to start the screening. Please note that the information you enter into this website is secure and cannot be seen or accessed by anyone other than the program employees who have invited you to participate in this screening.

By giving my consent, I understand that ASQ screening results will be shared with:

The Delaware Department of Education (DDOE) and the Division of Public Health (DPH) within Delaware Health and Social Services (DHSS) to better support my child.  

My child care program who is required by law to track completed developmental screenings to help better support my child.

The Birth to Three Early Intervention Program (B23) and my school district of residence because they review the screenings and help with the next steps. 

I will be contacted with results via email, US mail, or phone call to assist with follow up.
