Ages and Stages Questionnaire: Social-Emotional Screening

Help Me Grow Long Island's mission is to promote the optimal development of young children by:

·  linking families to information and community resources related to child development and parenting
·  providing personalized care coordination
·  creating and facilitating partnerships within the community to improve the availability and quality of services for families

Because your child's first 5 years of life are so important, we want to help you provide the best start for your child. You've been invited to participate in the Ages & Stages Questionnaires, Third Edition (ASQ-SE), to help you keep track of your child's social and emotional development. You will be asked to answer questions about some things your child can and cannot do. There is no right or wrong answer. After completing the screen, you will be contacted by a HMG-LI professional to review the results and discuss next steps to support your child.
