Welcome to Help Me Grow Maine's online screening program!

You are being offered to complete two tools to support your child’s development: the ASQ-3 and the ASQ:SE-2. Together, they provide a well-rounded view of your child’s growth. The ASQ-3 looks at communication, movement, problem-solving, and other key skills, while the ASQ:SE-2 focuses on social and emotional development. Both questionnaires are matched to your child’s age and offer a quick, helpful way to learn more about how your child is growing.

Because your child's first 5 years of life are so important, we want to help you provide the best start for your child. You've been invited to participate in the Ages & Stages Questionnaires: Social-Emotional, Second Edition (ASQ:SE-2) to help you keep track of your child's social and emotional development. The questionnaire may be provided for use at 2, 6, 12, 18, 24, 30, 36, 48, or 60 months of age. You will be asked to answer questions about some of your child's behaviors. The questionnaire includes questions about your child's ability to calm down, take direction and follow rules, communicate, perform daily activities (e.g., eating, sleeping), act independently, demonstrate feelings, and interact with others.

Please enter your child's birth date and the number of weeks he or she was born premature below to start the screening. Please note that the information you enter into this website is secure and cannot be seen or accessed by anyone other than the program employees who have invited you to participate in this screening.

Through Maine’s Office of Child and Family Services, Help Me Grow Maine offers families the option to share a copy of their child’s scored questionnaire with healthcare providers, schools, child care providers, or other community partners. This is a free and voluntary service that helps those who interact with your child provide more informed support and services. After you receive your child’s screening results, you will have the opportunity to choose which professionals or organizations, if any, you would like us to share this information with. No personally identifiable information will be shared without your consent.

 

In addition, Help Me Grow Maine may share summary, de-identified developmental screening data to support Maine’s broader public health efforts. Individual children cannot be identified through this shared data.
