List others assisting with completion of the questionnaire

As part of enrolling your child in preschool, we ask you to complete the Ages & Stages Questionnaire (ASQ). This is an important tool that helps us learn about your child’s development.

• You will fill out the questionnaire online and submit it through this Family Access system.

• By completing the ASQ, you give permission for your child’s preschool program and the Livingston Educational Service Agency (LESA) to share information with each other. LESA manages preschool enrollment for GSRP, PreK for All, and Head Start programs in Livingston County.

• All information will be kept private. After you complete the questionnaire, your child’s teacher will review the results with you.

• Your child’s results (without names attached) may also be combined with other children’s results. This helps improve preschool services and supports for all families in our county.
