We are committed to ensuring ALL children under the age of 6 in El Dorado County receive routine developmental screenings.  Because your child's first 5 years of life are so important, we want to help you provide the best start for your child. Ages & Stages Questionnaires (ASQs) can help you keep track of and support your child's development. The ASQ:3 includes questions about your child's communication, gross motor, fine motor, problem solving and personal social skills. The ASQ-SE:2 helps you understand your child's social and emotional development. 

Please allow yourself approximately 15 minutes to complete this questionnaire. There is not an option to save your progress and the questionnaire must be completed in one sitting. An alternative way is to print the questionnaires first and then enter the information online. Or you may ask our Family Engagement Specialist for hard-copy questionnaires. Please note that the information you enter into this website is secure and confidential.
