Our staff is committed to ensuring ALL children under the age of 6 in El Dorado County receive routine developmental screenings.

Because your child's first 5 years of life are so important, we want to help you provide the best start for your child. You are invited to participate in the Ages & Stages Questionnaire: Social-Emotional, Second Edition (ASQ: SE-2), to help you keep track of your child's development. You will be asked to answer questions about your child's social-emotional development, such as their ability and willingness to calm down, their ability to follow rules, and how they interact with others.

Please allow yourself 5-10 minutes to complete this questionnaire. There is not an option to save your progress and the questionnaire must be completed in one sitting. To begin, please enter your child's birth date. If your child was born premature, be sure to enter the number of weeks he or she was born premature before beginning the screening. Please note that the information you enter into this website is secure and cannot be seen or accessed by anyone other than the program employees who have invited you to participate in this screening.
